A sleepless nation 70 million strong

A new report by the National Academies' Institute of Medicine shows that between 50 and 70 million Americans struggle with chronic sleep problems, and according to the United States Surgeon General, sleep deprivation and disorders cost the nation $15 billion in health care expenses and $50 billion in lost productivity each year.


A lack of sleep can take a toll both psychologically and physically, from daytime sleepiness to weakened immune systems. Long-term sleep deprivation leads to problems that can include memory lapses, depression, irritability and impaired work performance.


To win the battle over sleepless nights, many Americans are turning to medication. In 2005, 42 million prescriptions for sleeping pills were filled. An analysis by Medco Health Solutions, Inc. found that the number of adults ages 20 to 44 using sleeping medications rose 128 percent between 2000 and 2005, and was up 114 percent among 10 to 19-year-olds in that same time period.


Insomnia is the most common sleep disorder, affecting between 30 and 40 percent of adults within a given year. Other sleep disorders include sleep apnea-a temporary suspension of breathing that occurs throughout the night-and restless leg syndrome, which is characterized by intense discomfort, mostly felt in the legs when at rest. One of the first steps to relieving sleep problems is to talk to your doctor or pharmacist about it.


Here are some tips for insomniacs:


• Keep to a regular sleep schedule: Go to sleep and wake up at about the same time each day, even on weekends.


• Exercise regularly: Exercise done early in the day can reduce stress and help put you to sleep at night.


• Watch what you eat and drink: Avoid alcohol, caffeine and meals close to bedtime, but a cup of warm milk can help make you sleepy.


• Unplug: This is particularly important for teens with sleep problems-make sure the bedroom is free of computers, video games, TVs and phones.


Sleep troubles can be a symptom of a physical or psychological condition that requires treatment, so it's important to see your doctor if your sleep problem persists. Depending on the diagnosis, your physician may recommend a behavioral therapy and possibly drug treatments for serious sleep disorders. Medications include both over-the-counter and prescription drugs called hypnotics. Drug therapy is usually only recommended for short-term use. A combination of medication and behavioral interventions is often more effective than either approach alone for those with chronic insomnia.


As many as one in four Americans report trouble sleeping.

Jean watched through the window

Jean watched through the window at the moon reflecting off the sea and the little children playing noisily on the beach. It was a temperate day and she felt pleased, reminiscences floated back to her from teenage years: when she had fallen in love with John, the weather had been in this way and she, as a coy young woman with big eyes and a head of vibrant red hair, had been delighted to catch the eye of, a young politician. Her lips that seemed to ask for a kiss had fascinated John as he walked though the park one day on his way to work, and despite the fact of his shyness his he had bought a box of chocolates – he said they were just like she was.

John never forgot her birthday and was always giving her nice flowers and chocolates and leaving loving notes around the house – even after 50 years of matrimony.


Her eyes brightened as she remembered some ardent moments – almost being caught by a farmer as they rolled in his haystack; skinny dipping in the warm Mediterranean Sea and relaxing days in hotel bedrooms when on holiday, scarcely leaving their room, so mad with desire were they.


A hand gently met her hand and she looked up: here he was, her John, now 72 but still with his brilliant eyes. He knew her so well that he without delay discovered what she was thinking.

Carpal tunnel syndrome - do you have it

Carpal tunnel syndrome reached epidemic proportions in the 90’s and has now achieved a status equaling pandemic proportions in the 21st century. With carpal tunnel syndrome having such devastating effects on millions of people each year, how do you know if you or someone you know has it?


With so much attention and hype the past 10-years concerning carpal tunnel syndrome in the workplace, you would think that a clear-cut picture would exist regarding the “how’s” “why’s” and “what is it’s” of carpal tunnel. Although carpal tunnel syndrome should be easily recognizable with such extensive media exposure, you would be amazed at how many people, including a lot of doctors, who do not recognize the symptoms or misdiagnose it as something other than carpal tunnel syndrome.


In order to provide a more clear picture of what carpal tunnel is, why and how it develops and what can be done to eliminate it, I have provided information below to explain the process so that you can better identify and therefore prevent carpal tunnel syndrome from taking over your life.


What is the Carpal Tunnel?


The carpal tunnel is a passageway in which the nine flexor tendons, median nerve, arteries, blood and lymphatic vessels pass through in order to supply function and movement to the fingers and wrist.


The carpal bones line the carpal tunnel on the posterior surface (backside) of the wrist with the transverse carpal ligament positioned on the anterior (front side) of the wrist. The size of the carpal tunnel is about the size of the index finger in diameter, and the flexor tendons, arteries and nerves glide past one another with ease in a carpal tunnel that has not decreased in size.


How Does Carpal Tunnel Syndrome Develop?


Because the finger and wrist muscles are constantly overused in one-way movement patterns, (Gripping, squeezing, typing, etc.) a "muscle imbalance" develops, causing the carpal bones to shift, in turn, making the carpal tunnel smaller and impinging the structures within, resulting in painful and debilitating symptoms.

Sensations involving tightness, discomfort, stiffness and pain on the front side of the hand and wrist may be present in carpal tunnel, but may also be symptoms of a general repetitive strain injury.


The only true telltale signs of carpal tunnel syndrome that you need to be aware of affect the thumb, index, middle and sometimes one-half of the ring finger. (All of the fingers and symptoms listed do not have to be experienced simultaneously for you to have carpal tunnel syndrome.)


If you have symptoms in your entire ring and/or little finger and your doctor tells you that you have carpal tunnel syndrome, you don’t! Create your own metabolism

Slow metabolism is frequently blamed for excess weight. The good news is we no longer have to be the victim of slow metabolism. We can actually be the creator of our metabolism. Knowing how your body works and using that knowledge to achieve your health and fitness goals, will get you results sooner with less frustration and aggravation. My goal is to help you get the most from your effort and see results.


How your metabolism works. Metabolism is the speed at which your body burns through food. As you get older, your metabolism decreases and you don’t require the same number of calories as when you were younger. The rate the metabolism decreases as we age is determined by the amount of muscle mass we have on our body. Greater muscle means higher metabolism. If you’re over 40, you probably aren’t as active as you used to be. You aren’t playing sports or chasing kids around the house. If you don’t use your muscle, you’ll lose it.


Digestion process burns calories. I teach my clients to eat 3 meals throughout the day (breakfast, lunch and dinner) and then an additional 2 to 3 healthy snacks between meals. Many of us have been conditioned to think that if we eat less, we will weigh less. The opposite is true but we must be careful to eat quality instead of quantity. There is the thermic value of nutrients — the actual calorie burning properties of nutrient digestion. This means that healthy, balanced eating offers a metabolic advantage. Eating is thermic or produces heat. Since a calorie is a measure of heat, balanced eating equals more calories expended.


Keep aerobic exercise moderate. Believing that aerobic exercise is the secret to burning fat is only half-correct. Too much aerobics subject us to a repetitive catabolic state where it literally is eating up muscle tissue. You can burn fat anytime you are in an “aerobic state.” If you are sitting and reading this article, you are in an aerobic state. You are in an aerobic state anytime you are meeting demands for oxygen. This is why proper nutrition and weight training is critical to maintain and build muscle. You can burn fat while you sleep with lean muscle mass. I use aerobic exercise to keep my heart, lungs and circulatory systems healthy and use weight training to burn calories and keep my metabolism healthy.


Muscle is metabolism. As we age, we begin to lose muscle. This is called muscle atrophy. The key is to preserve muscle to fight fat and keep your metabolism healthy. The muscle you have on your body is in direct correlation to the number of calories you burn at any given moment. Muscle doesn’t know age. You can maintain or build muscle at any age with weight training. I believe training with weights is more important as you get older because you are typically less active and your muscle will get soft from lack of use. Dense muscle mass is key to having a strong and healthy metabolism. Sandblasters and silicosis

What is Silicosis?


Silica is a compound formed from silicon and oxygen, often found in quartz, flint, agate, beach sand, sandstone and glass. When Silica particles are inhaled, it results in the formation of scar tissue in the lungs preventing oxygen from getting in the blood. This condition eventually results in a disease called Silicosis, an incurable and progressive lung disease. Approximately 300 people die from the disease every year in the United States.


More than one million workers are occupationally exposed to crystalline silica dusts in the United States. According to the World Health Organization, more than 100,000 of these workers are sandblasters. What is alarming is that 59,000 of them will eventually develop silicosis.


Sandblasting and Silicosis:


In Sandblasting, compressed air or steam is used to project a stream of abrasive particles onto a surface, which commonly includes materials like silica sand. Sandblasters or workers who operate this process are continually exposed to the harmful substance while cleaning sand and irregularities from foundry castings, removing paint etc because the silica sand used in abrasive blasting normally breaks up into fine particles and becomes airborne. Since these silica particles are invisible to the naked eye, workers may not be aware of the risks they are involved in, while performing their daily tasks.


In a research conducted by the National Institute for Occupational Safety and Health during the late 1990s, 99 sandblasters with cases of Silicosis were studied. Fourteen of them died from the disease. The remaining 85 developed complications related to Silicosis and were at risk of death. However, the relation between Silicosis and Sandblasting is not recent. As far back as in 1936, a study in Great Britain reported that 5.4% of a population of sandblasters died from silicosis or silicosis with tuberculosis during a 3 and a half year period.


Prevention of Silicosis for Sandblasters:


Avoiding inhalation of dust containing free crystalline silica is the first basic step to prevent development of Silicosis by people in the Sandblasting occupation. The use of crystalline silica for blast cleaning operations was already prohibited in Great Britain in 1950 followed by other European countries in 1966.


Adequate respiratory protection such as a type CE Abrasive Blasting Respirator should be used by everyone engaged in the sandblasting occupation, especially for workers adjacent to blasting operation machines.


Pneumatic (relating to air or other gases) tools used in industries like granite cutting tends to release larger quantities of free crystalline silica-containing dust. The use of these tools must be accompanied by an efficient dust control system.


Using safer materials such as Specular Hematite, Blasting Cullet, slag, or steel grit and shot as abrasive for sandblasting, instead of sand greatly decreases the chances of inhaling crystalline Silica particles.


Wetting down dry materials and surfaces before working on them also reduces the chances of crystalline Silica containing dusts becoming airborne. Eating, drinking, or smoking near sandblasting operations greatly increases the chances of crystalline Silica inhalation. Do wash your hands before. Do not take work clothes back home, which unnecessarily increases risks of exposure to your family and yourself.

Quitting smoking is not as hard as you think

Have you ever wanted to quit smoking and convinced yourself than you are destined to smoke cigarettes the rest of your life? Have you ever tried to quit and gave up halfway thought the day or that same night? If you think that quitting smoking is just impossible, I would read on.


It truly is not as hard as you may think. Take it from a true one packer a day smoker or better on some days. It was like a true friend to me that I couldn’t let go. I have smoked a good thirty to thirty six years of my life. I’m not quite sure when I started smoking. I quit one time about ten years ago for a week. The big mistake I made back then was to ask a friend for a smoke so I could see what I was missing. That was a big mistake! I was back full time again and tried several times since and failed on all attempts except the last attempt.


Quitting smoking for me was always like a ritual that I attempted two or three times a year. The excuse was always the cost of cigarettes. Never did I complain about the possible ill effects that it could possibly have down the road. Sure I was aware everyday about how bad it was. I heard it on the news every day on how the smokers were getting the boot in restaurants and buildings. The worst people to smoke around were the non-smokers that quit smoking. I know, my wife quit ten years ago. I smoked outside come rain, snow or blizzard. I hope as a non-smoker I never get like that.


I was not a very pleasant person to be around when I attempted to quit. I’m sure smokers know how short tempered one can get, when one has not had a cigarette for a while. Every morning I attempted to be a non-smoker, I set myself up for failure by having a coffee and shortly after breakfast I ran around outside hoping to bump into someone who smoked. I always had a nic fit around lunch and dinnertime, only to give in to the craving over and over again. I did not last a good part of a day. I just about gave up trying to quit. I was convinced that I was going to smoke the rest of my life. I thought I was hooked for good, until one sunny afternoon around 3:oopm I had my last cigarette while I was washing my jeep.


Everybody tells me that I was ready. I really don’t know if I was ready. What I do know is there are several key things that I had to do to give it up completely, and I am not really sure of the one key thing that kept me a non-smoker two months later. I know that one of the key things that helped was telling myself once and a while that I was a non-smoker even when I knew I smoked. I would not say it out loud if people were around me that knew I smoked. They would probably think I was losing it. I told myself that I was a non-smoker for about two weeks prior to quitting. Self-preparation you may want to call it. I truly knew that I wanted to quit. Maybe I was ready.


That sunny afternoon I had one cigarette left which I smoked halfway through washing my jeep. I told my wife that I was not going to buy any more that afternoon. She heard me say that many times before. She told me to give a try and purchase some later and not to be hard on myself for a least trying. Again she repeated it was okay to buy some later. I think my wife just about gave up on me quitting as well.


The other key thing to do if you are attempting to quit is to learn how to procrastinate. That same afternoon I kept putting off buying cigarettes until bedtime. Bedtime came and it was too late to go to the store. I put it off until the next morning.


Morning came and I convinced myself that I was a non-smoker. I decided the other key thing to do was invite my weaknesses. I had coffee, which they say you shouldn’t do. I almost stopped my brother-in-law from coming over that same night because he smoked. I told my wife to have him come over, but he had to smoke outside alone. I thought about it for a moment and decided I was going to stand outside and watch him smoke. They also said not to drink beer or alcohol when you’re attempting to quit smoking. I bought myself a six-pack and drank and watched him smoke. That was the very first day. I finally quit and had no craving the next day. The nic fits were gone! The cravings were gone! The grumpiness from not smoking was gone! The wasted dollars on cigarettes was gone! It was like being born again.


The one key goal on quitting smoking for me was to convince myself a couple weeks before, that I was a non-smoker. Quitting halfway through the day was the other key thing to do. Think about that for a second. The day for me was not as long as it would have been if I quit first thing in the morning. When you quit in the morning, you have approximately 16 hours to go before you go to bed and forget about it. I quit at 3:00 in the afternoon with only 8 hours until bedtime to forget about it. It was half the amount of time to put everything to rest. I had a 16-hour jump to get the nicotine out of my system before the morning. Quitting in the morning was always tough before, because smoking and a cup of coffee was always an enjoyment for me.


The last key thing to do was just learn to say no for the remainder of the next day. The craving was not as bad as it was on other days that I quit. It must have had something to do with the time I quit. The craving was not as bad as prior attempts. The next day I could not understand why I smoked all that time. The addiction is finally gone and I do not want to pick up that cigarette to see what I am missing!


Just convince yourself that you are a non-smoker for approximately two weeks.


Finish that last cigarette half way through the day when you are doing something that you enjoy. Make sure you are in a good mood!


Do not tell anybody that you’re quitting. Just tell them you are not buying any for the rest of the day.


Learn to procrastinate for 6 to 8 hours, which shouldn’t be too hard for some of us.


Face all your weaknesses that you think will put you back on the smoking train.


Say no for one day! Say no for one day! Say no for one day!


Day three will set you free!!!

Great tips for low carb dieters

If you have ever been on a 'diet,’ you will know how frightening that word is. Diet mean deprivation, right? Wrong. With a low-carb diet, there is no deprivation. In fact, you have the freedom to eat a lot of stuff that traditional diets do not allow. You only need to go light on the carbs! Carbohydrates (or carbs as they are lovingly called) are a type of complex sugar. They are ‘complex’ because they have a kind of wrapping that needs to be broken down by the body. Carbohydrates are found in almost any kind of food - bread, pasta, vegetables, and fruits. Carbs are stored in the liver where they are then broken down to form simple sugars called glucose. A diet rich in carbohydrates supplies a surplus of sugar to the body. This leads to obesity, diabetes, hypoglycemia, hypertension, and type II diabetes. Research has proved that the traditional low-fat, low-calorie diet is ineffective in reducing weight.


In a low carb diet, the carbohydrates in the diet are replaced with fat and proteins (poultry, cheese, meat, fish, and eggs; along with lots of vegetables). Carbohydrates should ideally make up only 5-10% of the daily calorie intake. The reasoning behind this diet is that carbs are readily converted into fat due to the action of insulin. Elevated insulin levels cause fat to be deposited in the body. So you become fat. High sugar in the blood also stimulates hunger signals in the brain. This is what produces the hunger cravings that lead you to binge on more carbs. So, it’s really a vicious circle, where the more you eat, the more you want.


The only way to put an end to this vicious cycle is by limiting the carb intake. Soon, the insulin levels start to fall and glucagons are produced. Now, glucagons are the nice guys - they burn fat and unclog the arteries. Low carb diets facilitate quicker weight loss and stabilize the blood sugar level (diabetics, are you listening?).


How to start a low carb diet:


-Consume lots of water as constipation may set in at the beginning of the diet


-Fiber and vitamin supplements need to be taken


-Sugar, pastries, and white flour are all taboo


-Avoid caffeine


-Exercise regularly


-Avoid saturated fats


A word of caution:


Many items in the market are marked “sugar free”. But these items are to be avoided. Sugar-free does not make it carb-free. Sugar-free cakes, biscuits and snacks still contain starch, which gets converted to sugar. Beware of the ‘no sugar added’ labels. This just means the manufacturer has not added any extra sugar to the product.


Fibers are good for you. Both soluble and insoluble fibers are necessary for good health.


It is quite normal for your body to become smaller without you losing weight. So, take your body measurements. Do not starve yourself or go hungry. Eat smaller meals and increase the frequency. Do not restrict the calorie intake, as this will slow down metabolism and force the body into ‘starvation mode.’ Instead, increase the fat and protein in your diet.


A diet without exercise is like a ‘10’ without the ‘1.’ Bump up your exercise level and add some weightlifting to your routine. Keep a diet diary, and meticulously and honestly keep records of what you consume.


Once the excess weight is gone, and your blood chemistry and energy levels have stabilized, you may start adding more carbs back into your diet.

Be good to your stomach

Our stomach is one of the parts of the body that receives the harshest treatment and the least appreciation. Think about it. When is the last time you were really thankful that your stomach was working properly? Perhaps it takes seeing a friend or family member who has a rough stomach for us to really be grateful for our stomach health. But it shouldn't. We should be consistently treating our stomach with care so that it can do it's job well without causing us pain.


Being good to your stomach can be simple. Maybe you are already making wise choices for your stomach or perhaps you will have to make some severe lifestyle changes in order to really be good to your stomach. Whatever it requires, you should commit to being good to your stomach from this point forward.


What does it mean to be good to your stomach? Basically, to be good to your stomach means watching the things you put into it and the things you do. Surely everyone has experienced the pain of eating a meal that is too spicy or too greasy or too full of acid. Our stomachs do so much in the process of breaking down our food choices. Many times our stomachs are able to cover up some of our eating "sins," but other times they let us know that we have made a poor choice. I'm convinced that we owe it to our stomachs (not to mention to the rest of our bodies) to make healthy eating choices that are easy on our bodies and good for our health.


Take some time to evaluate the things you are putting into your stomach each day. Are you being kind or harsh? Do you notice any patterns that could perhaps be harmful to your digestive system? If so, consider changing them. You only have one stomach, and therefore it requires your upmost attention and care. Different stomachs have different levels of tolerance for spices, acids, and foods. You may not be able to eat everything your neighbor or even your spouse can eat. It is important that you learn, if you haven't already, what is good for you and stick to it.


The great thing about learning to eat based on the reaction of your stomach is that you will end up making good choices not only for your stomach but also for the rest of your body. The chances are high that what is good for your stomach will also benefit your body and your overall health. So be intentional about what you eat and drink and don't hesitate to pass up things that aren't good for you or your stomach.

Who doesn't want glowing, vibrant, and younger looking skin? Everyone aspires to have a healthy and blemish-free skin. Still, many of us find our skin to be quite impossible to manage especially when you wake up and find a huge zit on your nose or a cold sore at the corner of your mouth. The good news is that there are ways to prevent and treat common skin problems by checking in the food we take.

Women are different from men in many ways. Men are perceived to be self-centered and egotistic while women are said to be more fragile and sensitive. These are the reasons why women's emotions are played differently in their hearts than in the hearts of men. However, these emotional problems can be surmounted if men treat women with more fairness and sensitivity. Stress management therapy is oftentimes necessary to counter the effects of these negative feelings. These negative feelings can be avoided if men treat women with more dignity and equality. These socio-cultural and relational adjustments from men may indeed help improve women's health in the long run. Mushy as it may sound, men should make their female counterparts feel that they are special, loved, and truly cared for.


The health benefits of 'worthwhile' relationships have been often taken for granted. Appreciation and love have taken the back seat in this era of technology. Lovers prefer to send e-mails, text messages, and instant messages rather than the old-fashioned love letters. “I love you's” are now more often 'texted' instead of being said in person. Gift-giving has become so routine and customary that it is only done during birthdays, valentine's day, and other special occasions. The act of giving gifts has lost its spontaneity, richness, and essence. Not too long ago, gifts were given as sign of love and passion. Men used to surprise women even with just simple gifts. The total unexpectedness of the gift only makes it more special for the women.


Men in intimate relationships should bear in mind that their counterparts need to be loved and respected. Treat them better, buy them flowers, or give them gifts every once in awhile. If she is surprised and asks you, “What's the occasion?” Just say, “Nothing...just because it's Friday.”

Canine companions cure

People normally encounter different situations that may cause more bad than good. Things like financial problems, death of a loved one, dealing with serious medical conditions, the breakup of a relationship, and certain changes in one's life can certainly cause depression. Having a family history of depression (with a pessimistic personality) can run in the genes for generations, and this too can affect one's lifestyle and well being if not treated as soon as possible.

With daily walks, dogs can help reduce the emotional symptoms while possibly helping other symptoms, such as fatigue or lack of energy.


Research has shown the many potential benefits of having a dog that can correlate to areas which health care professionals believe can help people in overcoming depression. Majority of pet owners feel that their pets are extremely important whenever they are feeling sad, lonely, or depressed. The “man's best friend,” can provide the desirable qualities as human such as listening, physical contact and empathy which helps in dealing with emotional symptoms of a depressed individual. Half of the population with dogs as companions believes that their pet makes a huge difference to their lives, ranking increased exercise and companionship as the two most important differences. Our pet dogs can help with the recovery of a depressed individual by relieving the stress and anxiety that the person may be experiencing. These dogs can really give a sense of comfort, purpose, and companionship that depressed people need in order to overcome their condition.

Big guy big girl does it indirectly mean fatty

If you are like me, and friends and family call big guy it usually means your are a bit overweight and indirectly means fatty. If you are sick and tired of it, it time to do something about it and prove some people you are not just a big guy but and intelligent one. Over the years being big guy has become a huge trend in sports. Multi billions of dollars spent in advertising featuring, you guessed it, big guys pitching you a new protein bar or super supplement. With all the supplementation products, and TV this for sure proves my point. If you look at an average person who is a bit overweight, he is considered the big guy of the group. Some will actually enjoy that title and some will not. Speaking from experience it is not too hot being called fatty indirectly.


Whenever you decide to take matters into actions and prove people that you can handle this whole big guy thing, you will start noticing peoples reactions immediately. Being big can mean problems though. Health problems that is. High cholesterol, Diabetes, Hypertension etc. Big guys usually encounter some of these or if not more diseases because of their size and shape. To avoid such problems, lets take are way of thinking and our outlook to life a bit differently. Start with eating better foods. Preferably greens and solid complex carbohydrates like; yams, sweet potatoes, grains etc. Greens will help you balance your PH acidic levels in your body, which will improve your health significantly and you will almost immediately notice your results.


Having a balanced Ph level means improving your health immediately. Ph of 7 or above means your blood is not as acidic therefore more healthy cells are produced and distributed to your organs. It has been known that controlling your body's alkalinity will give longevity and healthy organs.


Don't put the down the ever popular protein. Protein is the fuel to muscle gain and recovery. Although powders and pills are not necessary it may help speed the process. Increase your protein intake to about 1 gram per 1 pound of your weight. If you weight 185 lbs, then about 30 grams of protein in the spread of 6 meals for optimum metabolic boost. Protein may be underestimated now days because of its high impact from the media. But never abandon it. Best natural protein resources are from birds; chicken, turkey, fish; tuna, yellow fin, catfish etc. Stay away from red meat if possible, but if you still want red meat leaner cuts are very effective also.


If you stay consistent with these tips, you will gain muscle and eliminate extra weight you may be carrying. It is not easy to get started but once you start, like any habit you will create a life changing routines forever and disease will not face you. Do it for yourself, improve your well being and instead of being called a big guy you can be call a dude or girl with solid ripped abs.

The truth about infertility

Infertility is the inability to conceive; it may be remedied. However, should the conditions persist, it is referred to as sterility.


In the United States, one out of every 8 to 10 couples are childless because of infertility; it is a major medical and social problem. Both husband and wife are urged to seek medical attention for complete examinations and evaluation, since almost as many men as women cause a sterile marriage.


It is usually recommended that the male be evaluated first, because tests for the female are more expensive and time-consuming. Such tests may require the services of a urologist, gynecologist, endocrinologist, and internist.


In the female, the organs of reproduction and the glands influencing them are evaluated. Among the causative factors may be displacement and tumors of the uterus, genital infantilism, and inflammation. To allow fertilization of an ovum, it is necessary that the vagina, cervix, and uterus to be patent and have mucosal secretions receptive to the sperm. Semen is alkaline, as is cervical secretion; normal vaginal secretion is acid.


Treatment is directed towards correcting the deficiencies encountered in the individual patient. The following tests assist the gynecologist in delimiting the problem.


1. Rubin test


This procedure is to determine the potency of the fallopian tubes by introducing carbon dioxide through a sterile canula into the uterus, into the tubes, and into the peritoneal cavity.


By listening with a stethoscope on the abdomen, the physician may hear the gas swishing into the abdomen. If the pressure gauge reaches 200 mm. of a mercury, an occlusion may be suspected.


2. Salpinogram; Hysterosalpingogram


A radiopaque substance is often used to determine the site of tubal obstruction; an x-ray picture then shows the outline of the tubal lumen.


3. Hubner test


Within an hour or two after intercourse, the physician aspirates cervical secretions with a long cannula. The woman is to be instructed not to void, bathe, or douche between coitus and the examination; a perineal pad is worn until she is placed in “lithotomy position” in the examining room. Aspirated material is placed on a slide and examined under microscope for presence and viability of sperm cells.


Treatment


The treatment of sterility is a difficult matter, because it may be caused by a combination of several factors. Efforts are made to build up the general health of the patient, supplying lacking glandular hormones as indicated.


Operative treatments include removal of obstructions and plastic operations to restore tubal potency. Unfortunately, these are not too successful; in addition, there is a high incidence of tubal pregnancy following such operations.


The psychic and social factors of infertility must not be overlooked. Among some religious groups and nationalities, it is important to have blood descendants; adopting children is not acceptable in these families.


In other family groups, the man’s virility, or woman’s femininity are suspected when a couple fails to have children. Many pregnancies have occurred after an infertile couple has adopted a child, moved to another location, or the husband had a change of jobs.


All of these things indicate that the concepts behind infertility are not absolute and concrete. There are instances that the information about this particular problem may have underlying causes that should not be blamed on the inability of the person to conceive.


Boiled down, the problem on infertility is more of a social problem than a biological problem. It is not the aspect of not being able to conceive that persons involved are hesitant to disclose the matter.


If it is more than just biological malady, then, it requires tough solutions.

Bulimia

Bulimia is term used commonly for an eating disorder called Bulimia nervosa. It is a psychological condition in which a person engages in recurrent binge eating followed by intentionally doing one or more of the following in order to compensate for the intake of the food and prevent weight gain:


* vomiting


* inappropriate use of laxatives, enemas, diuretics or other medication


* excessive exercising


* fasting


The following six criteria should be met for a person to be diagnosed with bulimia.


1) The person feels incapable of controlling the urge to binge, even during the binge itself, and consumes a larger amount of food than a person would normally consume at one sitting.


2) The person purges him or herself of the recent intake, resorting to vomiting, laxatives, diuretics, exercising, etc.


3) The person engages in such behavior at least twice per week for three months.


4) The person is focused upon body image and desperate desire to appear thin.


5) The person does not meet the diagnostic criteria for anorexia nervosa. Some anorectics may demonstrate bulimic behaviors in their illness: binge-eating and purging themselves of food on a regular or infrequent basis at certain times during the course of their disease. Alternatively, some individuals might switch from having anorexia to having bulimia. The mortality rate for anorectics who practice bulimic behaviors is twice that of anorectics who do not.)


6) The person is of normal weight or overweight.


Bulimia is often less about food, and more to do with deep psychological issues and profound feelings of lack of control. Binge/purge episodes can be severe, sometimes involving rapid and out of control feeding that can stop when the sufferers are interrupted by another person or when their stomach hurts from over-extension. This cycle sometimes repeated several times a week or, in serious cases, several times a day. Sufferers often use the destructive eating pattern to gain control over their lives.

Simply follow the “slow, steady” rule when eating. It is better to eat six small meals spread throughout the day than consume three large meals. Don’t let yourself get hungry because the rise and fall in blood sugar can only add to your queasiness. Having gone without food for quite a while is the reason why many women feel nauseated in the morning. To avoid having an empty stomach, nibble on bland foods such as dry toast and saltine crackers. It is advisable if you can keep a box of crackers at your bedside to munch on before getting up in the morning.


Passing up foods that have strong odors and refraining from eating greasy or spicy foods help prevent morning sickness. It is also important for the pregnant woman to drink lots of water and to take prenatal vitamins.
